If you are desiring to go to the leadership workshop at University Hills Baptist Church in Charlotte on Tuesday, April 13, I need you to fill out this form and get it to me by noon on Monday, April 12. You can fax it (704-875-6581) or email it to me (mmims@fbc-h.org). Again, your cost is FREE and includes a meal. We will leave from the church at 5:15pm and return by 9:30pm.
